Katherine Tracy

Katherine E. Tracy, M.D., Medical Director

Dr. Tracy graduated from Boston College and received her Doctor of Medicine from St.George’s University School of Medicine. She has been with E.C.H.N. for over 25 years. In 1990 she was a full- time physician in the Emergency Department at Manchester Memorial Hospital. In 1998 she began working at CorpCare Occupational Health Center and became the Medical Director in 2012. She is board certified by the American Board of Internal Medicine. Dr. Tracy has 15 years of past experience serving as a consultant for the Department of Social Security and Disability.

Delythe Kracke

Delythe Kracke, A.P.R.N.

Ms. Kracke is a Nurse Practitioner who has been employed at CorpCare since 1997. Her responsibilities include performing pre-employment physical exams, diagnosis and treatment of the injured worker, providing follow-up care and working with the staff to provide comprehensive care in the occupational health setting. She received her Masters Degree in Nursing at the University of Connecticut. She maintains the APRN certification through ANCC and attends appropriate continuing education sessions as required by this credentialing organization.

Carol Holman

Carol J. Holman, Operations Manager

Carol’s LinkedInMs. Holman has been with CorpCare for 11 years. She started as a Certified Medical Assistant, then was soon promoted to Clinical Team Leader. Ms Holman became Operations Manager in 2007. As Operations Manager, Ms. Holman is in charge of the daily operations of the clinic. She attended Quinnebaug Valley Community College and Three Rivers Technical College.

Michelle Callahan

Michele Callahan, Secretary III

Ms. Callahan has been with CorpCare/ECHN since October 2008. She is responsible for all referral authorizations to outside specialists. Ms. Callahan coordinates a busy front desk. Ms. Callahan also assists with billing, and telephone triage.

Jennifer Conners

Jennifer Connors

Ms. Connors has been employed with CorpCare for 4 years. She currently works in the Secretarial role greeting patients and triaging initial injuries. She also maintains immunization and lab records in our software system, for hospital employees. Ms. Connors works with the clerical and clinical team to maintain patient flow and satisfaction. She comes to us from Connecticut Children’s Medical Center where she worked as an Emergency Technician. Ms. Connors has also worked as a Pharmacy Technician and has experience in billing.

Carol Curtain

Carol Curtin, M.A.

Ms. Curtin has been with ECHN for 26 years. She has worked as a Medical Assistant, Unit Secretary, Registrar, EKG Technician and Phlebotomist. She worked for 15 years in the Emergency Room/Prompt Care Departments and the Laboratory. She was with the Ambulatory Surgical Unit as a Medical Assistant for 10 years. Ms. Curtin transferred to Corp Care last year and currently works as a MA performing EKG’s, Phlebotomy, Visual Acutiy, Spirometry, Respiratory Fit Testing, Drug Screenings and assisting the physician as needed.

Jacquelyn Houghton

Jacquelyn Houghton

Ms. Houghton works as an Administrative Assistant for CorpCare Occupational Health. She has been with ECHN Rehab Services since 2011. Ms. Houghton assists the Physical Therapy Department with patient care, scheduling, and maintaining patient files. She also performs the work fitness assessments. Prior to joining ECHN she worked as an Administrative Assistant for a mental health facility.

Judith Hyde

Judith B. Hyde, RN, MSN

Ms. Hyde received her BS in Nursing from Russell Sage College in Troy, NY and her MSN from the University of Pennsylvania, Philadelphia, Pa. Ms Hyde has worked as a Nursing Supervisor for Manchester Memorial Hospital (ECHN). She is Chairman of the Committee to design the Nursing Clinical Ladder Program. Ms. Hyde is currently a Staff Nurse with CorpCare. She is a Certified Breath Alcohol Technician and a Mentor for Medical Assistant Externs. Ms Hyde also conducts offsite Flu, Hepatitis B and TB Screening Clinics. She is the recipient of ECHN Vision Award for Outstanding Healthcare Provider May 19, 2011 and a CPR Instructor.

Kylee Johnstone

Kylee Johnstone, M.A.

Ms. Johnstone started at Corp Care in 2007 as a Medical Assistant. She graduated from Porter and Chester Institute with a certificate in Medical Assisting. Her responsibilities at Corp Care include patient intake and triage along with phlebotomy, spirometry, audiograms and drug screens. Ms Johnstone is a certified Breath Alcohol Technician and CPR certified.

Brian Wright

He is responsible for new business development and is the point of contact between business clients and CorpCare’s clinical staff.

Sarah Laureano, R.T.R.

Ms. Laureano has been an X-ray Technologist at Corp Care for six years. She has been with ECHN for ten years. Ms. Laureano is responsible for performing X-rays, phlebotomy, audiograms, spirometry, EKG’s, vital signs and drug screening.

Lynn Lord

Lynn Lord, Customer Service Rep

Ms. Lord joined ECHN CorpCare in 1998. She is responsible for all the billing and insurance information. Ms. Lord is responsible for entering crucial work status information into our software system. This allows CorpCare to produce Injury Analysis reports. Ms. Lord works with client companies to build their profiles adding tasks and procedures. She is backup for the front office, doing scheduling and patient intake.

Alexander Smith

Alexander B. Smith, MD, MS, FACOEM,

Dr. Smith graduated from Brown University in 1969, received his Doctor of Medicine from Johns Hopkins University School of Medicine in 1973, and his Master of Science degree from the University of Cincinnati Kettering Institute in 1980. He has been board-certified in Occupational Medicine since 1982. He is a Fellow of the American College of Occupational and Environmental Medicine. From 1975 to 1995 he was a commissioned officer with the United States Public Health Service. In that capacity, he served with the National Institute for Occupational Safety and Health (1976-1988), National Oceanic and Atmospheric Administration (1988-1991), and United States Coast Guard (1991-1995). From 1996 to 2011, he was Medical Director of CorpCare Occupational Health Center in Manchester, Connecticut. He continues to work part-time for the Connecticut Occupational Medicine Partners clinic system.

Jenna Stemmerman

Jenna Stemmerman, R.T.R.

Ms Stemmerman has been a radiologic technologist (x-ray technologist.) at CorpCare Occupational Health for two years. In addition to her x-ray responsibilities at CorpCare, she also performs drug screens, audiology test, phlebotomy, spirometry test, and respirator fit tests. She graduated magna cum laude with an Associates Degree of Science in Radiologic Technology at Capital Community College in 2009. Currently, Ms Stemmerman is a Connecticut State licensed radiographer, nationally board registered through The American Registry of Radiologic Technologists (ARRT) and CPR certified. Prior to CorpCare, she was a volunteer EMT in Tolland.

Louise Caudullo

Louise Caudullo, D.O.

Dr. Caudullo currently works at the Corp-Care Occupational Health office and is the site physician at JCPenney -Manchester Occupational Health office. She has been employed as a staff physician with Eastern Connecticut Health Network since 1999. Dr. Caudullo is a graduate of the New York College of Osteopathic Medicine in Old Westbury, NY. She did her residency in Emergency Medicine at PCOM/Albert Einstein Hospital in Philadelphia Pa. Dr. Caudullo is AOBEM Board Certified in Emergency Medicine. She is a member of the American Osteopathic Association and American Osteopathic Board of Emergency Medicine.
